The process of finding appropriate in-home care can be confusing and
stressful; the sheer breadth of information available and the
uncertainty of what action to take is often overwhelming. 

To make matters worse, many agencies pile layer upon layer of different staffers, coordinators, intake specialists and so on..which just makes the process even harder.

Our care system is streamlined and uncomplicated, with the singular goal of insuring that the care recipient is secure, healthy and enjoying life to the fullest.  Determing the c
orrect home care solution for you or a loved one doesn't have to be difficult or burdensome.

STEP 1:
Preliminary Acquaintance
When you first contact AngeliCare, we start the process by building a profile of the client.  This involves basic questions about the client's health history, care requirements and client and family objectives.
This preparative information is essential in identifying the client's care needs and whether or not our services can meet those care needs.

STEP 2: In-Home Assessment



Once we have determined that our service can be of benefit to the client, the next step involves the scheduling of an in-home assessment.  You can take comfort in knowing that the care specialist that you initially talked with will be the same person that comes to you home and performs the assessment.

 
The assessment is an in-depth, fact gathering consultation that is the foundation in creating a customized care plan specific to meeting the client's and families care requirements and health goals. 
To insure that our clients are provided with the best possible care, we cannot provide service of any nature without a preliminary in-home assessment.  Assessment's remain valid for 6 months.  If you are thinking about in-home care, or know that you will be needing care in the near future (e.g. hospital procedure, rehabilitation, respite care, loss of primary caregiver, etc. ..), we encourage that you schedule an assessment ASAP, or at minimum, prior to the estimated time-frame of need.  Once the assessment is completed, we can provide immediate help when notified.

  At AngeliCare, assessments are always without cost or obligation.

STEP 3: Customized Care Plan
The specifics gathered from the assessment are utilized in creation of the Care Plan. The Care Plan involves the scheduling and appointment of caregivers to provide the assistance needed in helping meet the client's care or social needs. The services that our professional caregivers provide include Personal Care, Companion Care, Homemaker Services and Specialty Services.  Listed below is a brief example of the services we provide:

STEP 4: Selection of Care Providers

The information acquired from assessment is also instrumental in the selection of caregivers for our clients. Our state-of-the-art software assists us in matching caregivers to clients based variables such as  the client's care needs, personality traits (client and caregiver), caregiver's skill sets and life experiences. This is an extremely important part of the process as compatibility is the key ingredient to insuring a positive, successful in-home care experience.

If the match isn't as successful as anticipated, then we'll replace the caregiver with another. We will continue to "tweak" the selection until the client and/or their family is satisfied.  Our caregivers are experienced, understanding individuals and they understand that sometimes personalities don't mesh. 

STEP 5: Continual Care Management

As with all aspects of life, health and subsequent care needs change.  This is especially true as we age.  One of the major advantages of our care program is Continual Care Management. Our caregivers are trained and dutifully required to observe, monitor and document all health related aspects of their clients. We consistently inform and report all noted health related concerns to family members, primary care physicians and appointed guardians of noted changes or issues. Documentation is always available and accessible at the client's home via their Care Plan Book.

When or if the client's care requirements do change, our care process is highly flexible and we can easily revise the schedule and/or level of care needed to insure that the client's care needs are met.  If we determine that the client's care needs have surpassed our service levels, we immediately notify the primary guardian or family of situation. In those situations, you can be assured that AngeliCare will do everything can to help guide you to the proper level of care needed for you or your loved one.
